There is no greater smoke-and-mirrors company involved in TV 2.0 than Spot Runner. These guys claim to automate the production of TV commercials - in fact, they're a pretty basic production company with a good PR company and an online form. In a pretty useless and highly fragmented marketplace you can't blame them for trying.
Visible World is another company involved in producing bespoke ads - and they can deliver.
However, Flash and WPF/E (sorry, Silverlight..) are capable of delivering totally dynamic ads, so these providers may quickly become irrelevant.
